I use this when he’s kicking on his back on his playmat, changing table or in his Pack n’ Play and I am keeping a close eye. I have also used when he’s laid back in a stroller. At 4 months his head was still flat but better than at 2 months. We used this pillow while he was in his boppy, his soother swing and at night. It does lift his head which makes his chin come more to his chest which I didn’t like. Around 3.5 months he became active enough that he would roll off of it. I do believe it helped. It didn’t solve our problems but it did help.

This pillow really helps alliviate flat head syndrome until your baby is more efficient at tummy time and can spend longer time on tummy as opposed to back. Of course, you should have your pediatrician take a look at the actual flat head on your baby to check if it's something more serious like torticollis that actually need pt help.
